It's where poop comes out, so obviously there are going to be certain risks. It's an area of the body that is rich in bacteria, fungus and all kinds of microbes, and even though a shower or an enema helps, they don't eliminate everything.

There's risk for shigella and other diseases related to fecal matter, like pink eye.

But it's not absurdly risky or even likely, or people wouldn't do it at all.
If you want to play, you've got to take on certain risks, unfortunately.

I am the who is being rimmed. The only contact that I have is with this person's mouth and saliva. Isn't it more risky for him?

You could catch Ass-and-Mouth disease. It’s like Foot-and-Mouth disease, but not as bovine. The virus causes a high fever for between two and six days, followed by blisters inside the mouth and on the ass that may rupture and cause lameness. It is highly infectious and can be spread by infected hosts comparatively easily through contact with contaminated farming equipment, vehicles, clothing, food, and by domestic and wild predators.
